The result of intensive work by the Commission on end-of-life care,\nthe 127 pages and appendices of the five-year report received limited media\ncoverage due to the abundance of news items this week.<o:p><\/o:p><\/span><\/p><p style=\"text-align:justify\"><span lang=\"EN-CA\" style=\"font-family:&quot;Arial&quot;,sans-serif;mso-ansi-language:EN-CA\">The Living with\nDignity citizen network (LWD) invites the public to read this important work,\nwhich is available in French at <\/span><a href=\"https:\/\/csfv.gouv.qc.ca\/publications\"><span lang=\"EN-CA\" style=\"font-family:\n&quot;Arial&quot;,sans-serif;mso-ansi-language:EN-CA\">https:\/\/csfv.gouv.qc.<\/span><\/a><a href=\"https:\/\/csfv.gouv.qc.ca\/publications\"><span lang=\"EN-CA\" style=\"font-family:\n&quot;Arial&quot;,sans-serif;mso-ansi-language:EN-CA\">ca\/publications<\/span><\/a><span style=\"font-family:&quot;Arial&quot;,sans-serif;mso-ansi-language:EN-CA\"> <span lang=\"EN-CA\">(with highlights available in English) as well as the document <a href=\"https:\/\/vivredignite.org\/sites\/vivredignite.org\/wp-content\/uploads\/CSFV_Rapport-2018-2023_Regard-faits-saillants.pdf\">Regard\nsur les faits saillants du Rapport quinquennal 2018-2023 sur la situation des\nsoins de fin de vie<\/a> (A look at the highlights of the 2018-2023 Five-year\nReport on the state of end-of-life care) prepared in French by Dr. Michel\nBureau, outgoing president of the Commission on end-of-life care (thank you to\nthe Commission for allowing us to share it with as many people as possible).<o:p><\/o:p><\/span><\/span><\/p><p style=\"text-align:justify\"><span lang=\"EN-CA\" style=\"font-family:&quot;Arial&quot;,sans-serif;mso-ansi-language:EN-CA\">After a first\nreading of the documents, LWD would first like to focus on the section on\npalliative care, which is very rich in information. The citizen network will\naddress the other themes of the report, including that of medical aid in dying\n(MAiD), in an upcoming second press release. <o:p><\/o:p><\/span><\/p><p style=\"text-align:justify\"><span lang=\"EN-CA\" style=\"font-family:&quot;Arial&quot;,sans-serif;mso-ansi-language:EN-CA\"><br><\/span><\/p><p style=\"text-align:justify\"><b><span lang=\"EN-CA\" style=\"font-family:&quot;Arial&quot;,sans-serif;mso-ansi-language:EN-CA\">A glaring lack\nof data to paint an adequate picture of palliative care<\/span><\/b><span lang=\"EN-CA\" style=\"font-family:&quot;Arial&quot;,sans-serif;mso-ansi-language:EN-CA\"><o:p><\/o:p><\/span><\/p><p style=\"text-align:justify\"><span lang=\"EN-CA\" style=\"font-family:&quot;Arial&quot;,sans-serif;mso-ansi-language:EN-CA\">As the\nCommission&#8217;s 2023-2024 report on end-of-life care did not address the issue of\npalliative care, Living with Dignity is happy to see that the five-year report\ndeals with it directly (pages 8 to 15). One major observation clearly emerges:\nthe available data \u201cdo not make it possible to ascertain the true level of\naccess to palliative care in the network or the quality of the care provided,\nin particular whether needs are effectively met and what the obstacles are to\naccessing timely, high-quality palliative care.\u201d (quotes from the report are\ntranslated by LWD).<o:p><\/o:p><\/span><\/p><p style=\"text-align:justify\"><span lang=\"EN-CA\" style=\"font-family:&quot;Arial&quot;,sans-serif;mso-ansi-language:EN-CA\">This information\ncasts a shadow over the statement that \u201cnearly 68% of people who received MAiD\nwere receiving palliative and end-of-life care at the time of the assessment of\ntheir request, and 82% received it between that time and the administration of\nMAiD. These proportions are lower than those reported in the Commission&#8217;s\n2015-2018 triennial report (80% and 89%, respectively).\u201d The question therefore\narises: what proportion of these people had access to comprehensive palliative\ncare at the right time and with an interdisciplinary approach that was <i>biological,\npsychological, social and spiritual<\/i>? A purely pharmacological approach\n(such as the simple administration of morphine) could be described as\npalliative care, but that is a highly simplistic view. For a real choice at the\nend of life, Quebecers must have access to palliative care worthy of the name\nthat adequately addresses physical, psychological and existential suffering.<o:p><\/o:p><\/span><\/p><p style=\"text-align:justify\"><span lang=\"EN-CA\" style=\"font-family:&quot;Arial&quot;,sans-serif;mso-ansi-language:EN-CA\">Among the other gaps\nidentified by the Commission, it should be noted that \u201cthe data collected does\nnot allow for identification of the people who could have or should have\nreceived palliative care because of their illness or condition. This\ninformation can only be collected indirectly, based on terminal illness\ndiagnoses. Better information on this subject would make it possible to better\nassess whether the palliative care provided corresponded to the patients\u2019 needs\nas well as, possibly, the reasons for a lack of correspondence (e.g. late\nreferral, insufficient resources, refusal by the person).\u201d<o:p><\/o:p><\/span><\/p><p style=\"text-align:justify\"><span lang=\"EN-CA\" style=\"font-family:&quot;Arial&quot;,sans-serif;mso-ansi-language:EN-CA\">&nbsp;<\/span><\/p><p style=\"text-align:justify\"><b><span lang=\"EN-CA\" style=\"font-family:&quot;Arial&quot;,sans-serif;mso-ansi-language:EN-CA\">Do some people\nchoose medical aid in dying because of a lack of adequate palliative care?<\/span><\/b><span lang=\"EN-CA\" style=\"font-family:&quot;Arial&quot;,sans-serif;mso-ansi-language:EN-CA\"><o:p><\/o:p><\/span><\/p><p style=\"text-align:justify\"><span lang=\"EN-CA\" style=\"font-family:&quot;Arial&quot;,sans-serif;mso-ansi-language:EN-CA\">We agree with\nDr. Bureau&#8217;s analysis in his \u201cRegards sur les faits saillants\u201d (on page 7):\n\u201cThe Commission&#8217;s 2018-2023 report does not provide a direct answer to this\nquestion.\u201d<o:p><\/o:p><\/span><\/p><p style=\"text-align:justify\"><span lang=\"EN-CA\" style=\"font-family:&quot;Arial&quot;,sans-serif;mso-ansi-language:EN-CA\">We welcome, as\nhe does, the \u201csteady increase in the number of people receiving palliative care\nat home, in palliative care homes and in health care establishments (including CHSLDs)\u201d.\nEvery effort must now be made to ensure that it is accessible as early as\npossible in a care pathway and with an interdisciplinary approach.<o:p><\/o:p><\/span><\/p><p style=\"text-align:justify\"><span lang=\"EN-CA\" style=\"font-family:&quot;Arial&quot;,sans-serif;mso-ansi-language:EN-CA\">Living with\nDignity would like to commend the work of the Commission, chaired until October\n9 by Dr. Michel Bureau and now by Dr. Lucie Poitras.
