How many safeguards do you need to make assisted suicide safe?

The Final Report of the Ministerial Advisory Panel on Assisted Dying set up by Victorian Premier, Daniel Andrews late in 2016, recently handed down its final report and recommendations. After reading the report, it is hard to suggest that there can be such a thing as safe suicide.
